Job purpose/summary: This is a 37.5 hour full time permanent role. We are looking for a Senior Diabetes Nurse Specialist, or ACP with a Special Interest in Diabetes, to lead our Diabetes Team at Health and Beyond this includes supervising Diabetes care provision across all of our sites including the Primary Care Network (PCN). You will work with the Diabetes Nurse Specialist to manage our patients living with Diabetes across all sites also working alongside our management, clinical and admin teams with a view to improving our patients health outcomes. You will be able to provide management, assessment, screening and education to Diabetic patients running specific clinic sessions and maintaining robust call and recall systems. Qualifications required: Qualified RGN or ACP registered with the NMC with at least 4-5 years post registration experience in the field of Diabetes preferably in Primary Care. Diabetes Diploma e.g. Warwick Diploma in Improving Diabetes or similar accredited Diabetes qualification. Nurse Independent Prescriber (V300) Qualification in Leadership and Management. Qualification in Teaching and Assessment in clinical practice. Documented Continual Professional Development. Knowledge: Extensive knowledge of Diabetes Care. Management of complex Diabetes and Diabetes complications. Up to date knowledge of Diabetes Technology Knowledge of up to date research in Diabetes Experience: Extensive post registration experience working as a Diabetes Nurse Specialist Experience of working autonomously Leadership experience Skills and ability: Excellent organisational and clinical skills for this role. Ability to teach groups of patients and staff Ability to Organising own specialist workload. Leadership skills Excellent communication skills Adaptable and good teamwork. Excellent IT skills excellent use of WORD, Excel and PowerPoint Ability to act as an effective resource and expert in Diabetes and actively disseminate relevant information to both medical and nursing staff members within the multidisciplinary team and other health professionals. Ensure that the service is provided within the agreed resource envelope, escalating concerns regarding the Diabetes team meeting expected outcomes including QOF. Job Responsibilities: Clinical Nursing Practice Work as part of a multi-disciplinary team to deliver safe and high-quality care. Lead the Diabetes team to carry out annual reviews and consultations of patients with Diabetes including medication review and improve access for this cohort to physical health interventions. Work with the MDT to support and advise the practice team in triage, assessment and treatment of patients needing support with their Diabetes Carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care. Delivery of care in line with NSF, NICE guidelines and evidence-based care Perform specialist assessment of Diabetic patient needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate. Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved. Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice. Participate in regular review of prescribing practice with prescribing mentor. Manage Diabetic patients care, including drug and non-drug-based treatment methods, and formulate this into a management plan. Request diagnostic investigations and interpret within agreed protocols e.g. blood tests. Review and treat patients in practice premises, by telephone, or in their own home who require medical attention/review and/or refer to appropriate agency. Refer patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary. Care Management Communicate confidential and sensitive information to patients, relatives and carers in relation to their condition. Work closely with colleagues in specialist community Diabetes services to ensure smooth transitions between teams and services and facilitate an improved access to evidence-based interventions Work closely with, and provide day to day support and guidance to, the Care Co-ordinators and Health Coaches to connect patients to appropriate community and voluntary sector support. Oversee and facilitate Diabetic assessments in practice and support the practice team in accessing appropriate level of services. Provide highly specialised care advice beyond the scope of normal Nursing practice e.g. care treatment plans, making referrals as appropriate (including fast track referrals). Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme, National benchmarking. To monitor and meet care outcomes against standards and recommend or initiate changes as necessary. Health Improvement Management of health screening programmes and ensure other team members are aware of health priorities and screening objectives and programmes. Innovation in practice to meet ongoing demands on the practices delivery of care. Improve health outcomes for patients by introducing new developments, and can demonstrate them as reports and presentations where appropriate. Supports practitioners in reporting the practice population to promote health improvement and disease prevention. Makes referrals to appropriate agencies (statutory and voluntary) where necessary Providing assessment, screening services, health education advice: These may include (but not restricted to); Diabetic Reviews Blood pressure check/reviews Medication Reviews Administration and professional responsibilities Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures Participate in the administrative and professional responsibilities of the practice team Accurate and legible notes of all consultations and treatments are recorded in the patients notes Ensure clinical systems kept up to date, recording and/or amending accurate details Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice Lead Diabetes meetings and participate in practice and nurse meetings as required Restocking and maintenance of clinical areas and consulting rooms as required. Training and personal development Maintain up to date skills and knowledge, maintaining awareness of professional issues at an advanced level. Training needs will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. Assess effectiveness of care delivery through self-reflection and peer review, as well as benchmarking and formal evaluation. Contribute to the identification and assessment of learning needs of staff and other professionals and assist in planning effective programmes of education. Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate. Act as a mentor for own team members and other staff, assessing competence against set standards. Take responsibility for own and teams development, learning and performance and demonstrate skills and activities to others who are undertaking similar work and ensure high standards of own educational commitment always being mindful of professional revalidation requirements. Develop and maintain a Personal Learning Plan. Regularly participate in clinical supervision. Leadership personal and people development Act as a clinical leader in the delivery of not only Diabetes but also practice nursing services ensuring that the needs of the patient are a priority Support staff development in order to maximise potential Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Critically evaluate and review innovations and developments that are relevant to the area of work Work with practice management/Lead Nurse to ensure sufficient staffing to provide a safe service, and that staff have the appropriate ability, quality and skill-mix to meet current and future service delivery Take part in recruitment processes where appropriate. Maintain effective communication and active involvement in the planning and processes of practice-based commissioning or similar initiatives Promote the role of the Senior Diabetes Nurse Specialist in General practice.
